Individual Care
An ongoing nursing relationship centered on your health, goals, and daily life. Begin with a comprehensive assessment and an individualized plan of care, followed by scheduled visits and check-ins to monitor progress, address concerns, and refine your plan.
Family Care
Shared family goals with room for individual needs. A family assessment establishes your household’s priorities and plan of care. Pooled follow-up visits support participating adults and children, including focused nursing care for non-emergent temporary concerns, with individual consent and privacy.
Initial Consultation
A focused consultation to explore your health concerns, clarify priorities, and help determine the most useful next step. This visit offers further assessment before choosing an annual Individual or Family plan.
